This change replaces [gorp] with [borp].
The changes consist of a mass renaming of the import and comments / doc
fixups, plus modifications of many call sites to provide a
context.Context everywhere, since gorp newly requires this (this was one
of the motivating factors for the borp fork).
This also refactors `github.com/letsencrypt/boulder/db.WrappedMap` and
`github.com/letsencrypt/boulder/db.Transaction` to not embed their
underlying gorp/borp objects, but to have them as plain fields. This
ensures that we can only call methods on them that are specifically
implemented in `github.com/letsencrypt/boulder/db`, so we don't miss
wrapping any. This required introducing a `NewWrappedMap` method along
with accessors `SQLDb()` and `BorpDB()` to get at the internal fields
during metrics and logging setup.

Add an upstream ProxySQL container to our docker-compose. Configure
ProxySQL to manage database connections for our unit and integration
tests.

We have decided that we don't like the if err := call(); err != nil
syntax, because it creates confusing scopes, but we have not cleaned up
all existing instances of that syntax. However, we have now found a
case where that syntax enables a bug: It caused readers to believe that
a later err = call() statement was assigning to an already-declared err
in the local scope, when in fact it was assigning to an
already-declared err in the parent scope of a closure. This caused our
ineffassign and staticcheck linters to be unable to analyze the
lifetime of the err variable, and so they did not complain when we
never checked the actual value of that error.
This change standardizes on the two-line error checking syntax
everywhere, so that we can more easily ensure that our linters are
correctly analyzing all error assignments.

This also causes a behavior change in the VA. If a HTTP connection is
abruptly closed after serving the headers for a non-200 response, the
reported error will be the read failure instead of the non-200.
Also:
- Use MockCA in the RA test instead of a real CA.
- Since the mock CA doesn't write to an SA, remove a part of the RA test that
checked that the certificate was written. That code is already tested in the CA,
where the test belongs.
- Format the constants in RA test to be more copy-and-pasteable.
- Remove Printf in mocks/log.go and test/db.go to make failed test output more readable.

Also removes currently-unused 'development' DB, and do initial migrations in
parallel, which shortens create_db.sh from 20 seconds to 10 seconds.
Changes ResetTestDatabase into two functions, one each for SA and Policy DBs,
which take care of setting up the DB connection using a special higher-privileged
user called test_setup.
This has required some substantive changes to the tests. Where
previously the foreign key constraints did not exist in the tests, now
that we use the actual production schema, they do. This has mostly led
to having to create real Registrations in the sa, ca, and ra tests. Long
term, it would be nice to fake this out better instead of needing a real
sa in the ca and ra tests.
The "goose" being referred to is <https://bitbucket.org/liamstask/goose>.
Database migrations are stored in a _db directory inside the relevant
owner service (namely, ca/_db, and sa/_db, today).
An example of migrating up with goose:
goose -path ./sa/_db -env test up
An example of creating a new migration with goose:
goose -path ./sa/_db -env test create NameOfNewMigration sql
Notice the "sql" at the end. It would be easier for us to manage sql
migrations. I would like us to stick to only them. In case we do use Go
migrations in the future, the underscore at the beginning of "_db" will
at least prevent build errors when using "..." with goose-created Go
files. Goose-created Go migrations do not compile with the go tool but
only with goose.
