- Upstream CFSSL update broke a test in certificate-authority_test.go
  See https://github.com/cloudflare/cfssl/issues/156
- Added "SerialPrefix" to config, letting you set a prefix to the generated
  serial numbers.

Peter Eckersley 5eac0cda09 Add a "TestMode" config option
