Simplify profile config hashing (#8081)
Remove the backwards-compatible profile hashing code. It is no longer
necessary, since all deployed profile configs now set
IncludeCRLDistributionPoints to true and set the UnsplitIssuance flag to
true. Catch up the CA and crl-updater configs to match config-next and
what is actively deployed in prod.

ca: unsplit issuance flow (#8014)
Add a new RPC to the CA: `IssueCertificate` covers issuance of both the
precertificate and the final certificate. In between, it calls out to
the RA's new method `GetSCTs`.

The RA calls the new `CA.IssueCertificate` if the `UnsplitIssuance`
feature flag is true.

The RA had a metric that counted certificates by profile name and hash.
Since the RA doesn't receive a profile hash in the new flow, simply
record the total number of issuances.

issuance: add CRLDistributionPoints to certs (#7974)
The CRLDP is included only when the profile's
IncludeCRLDistributionPoints field is true.

Introduce a new config field for issuers, CRLShards. If
IncludeCRLDistributionPoints is true and this is zero, issuance will
error.

The CRL shard is assigned at issuance time based on the (random) low
bits of the serial number.

issuance: add new IncludeCRLDistributionPoints bool (#7985)
To achieve this without breaking hashes of deployed configs, create a
ProfileConfigNew containing the new field (and removing some deprecated
fields).

Move the CA's profile-hashing logic into the `issuance` package, and
gate it on the presence of IncludeCRLDistributionPoints. If that field
is false (the default), create an instance of the old `ProfileConfig`
with the appropriate values and encode/hash that instead.

Note: the IncludeCRLDistributionPoints field does not yet control any
behavior. That will be part of #7974.

Remove unused keys from CA config (#7948)
Remove the singular Profile field from the CA config, as it has been
replaced by the plural CertProfiles key. Remove the Expiry, Backdate,
LintConfig, and IgnoredLints keys from the top-level CA config, as they
are now also configured on a per-profile basis. Remove the LifespanCRL
key from the CA config, as it is now configured within the CRLProfile.
For all of the above, remove transitional fallbacks from within
//ca/main.go.

These config changes were deployed to production in IN-10568, IN-10506,
and IN-10045.

Introduce SerialPrefixHex field in CA (#7721)
Add a new SerialPrefixHex field to the CA's config, which takes a
two-character hexadecimal string to use as the serial prefix. This
matches the way that the OCSP Responder's acceptable serial prefixes are
configured, and is easier for human operators to configure than raw
integers.

At the same time, change the type of the CA's internal serial prefix
from `int` to `byte`, using the type system to enforce its 8-bit length.

Configure lints separately for each profile (#7636)
Move the two lint-configuration keys, LintConfig and IgnoreLints, from
the top-level CA.Issuance config stanza into each individual
CA.Issuance.CertProfiles stanza. This allows us to have
differently-configured lints for different profiles, to ensure that our
linting regime is as strict as possible.

Without this change, it would be necessary for us to ignore both the
"common name included" and the "no subject key id" lints at the
top-level, when in fact each of those warnings only triggers on one of
our two profiles.

Use profile to determine backdate and validity (#7621)
One of our goals with profiles is to allow different profiles to have
different validity periods. While the profiles already had the ability
to enforce different maximum backdates and validities, the CA still had
separate global configuration for what the backdate and validity period
should actually be.

Move the computation of the notBefore and notAfter timestamps into the
issuance package, so that it can be based on the profile's configured
backdate and validity durations. Deprecate the global "backdate" and
"expiry" config fields, as they are no longer used. Finally, add more
validation for the profile's backdate and validity.

KeyPolicy: add custom constructor and make all fields private (#7543)
Change how goodkey.KeyPolicy keeps track of allowed RSA and ECDSA key
sizes, to make it slightly more flexible while still retaining the very
locked-down allowlist of only 6 acceptable key sizes (RSA 2048, 3076,
and 4092, and ECDSA P256, P384, and P521). Add a new constructor which
takes in a collection of allowed key sizes, so that users of the goodkey
package can customize which keys they accept. Rename the existing
constructor to make it clear that it uses hardcoded default values.

With these new constructors available, make all of the goodkey.KeyPolicy
member fields private, so that a KeyPolicy can only be built via these
constructors.

Fix reference bug in CA.noteSignError (#7534)
In the process of writing
https://github.com/letsencrypt/boulder/pull/7533 I discovered that the
method for detecting pkcs11.Error errors is broken: it attempts to
unwrap the returned error into a pointer-to-a-pointer type, which
doesn't work because only `pkcs11.Error` implements the Error interface,
while `*pkcs11.Error` does not.

Add a test which shows that the current noteSignError implementation is
broken. Then fix noteSignError and the two locations which duplicate
that code by removing the extra layer of indirection. And since the same
code exists in three locations, refactor how the caImpl, ocspImpl, and
crlImpl share metrics so that it only has to exist in one place.

Add signature count and error metrics to crlImpl (#7533)
Add the "signatureCount" and "signErrorCount" metrics, which are already
incremented by the certificateAuthorityImpl and ocspImpl after all
signing operations, to the crlImpl.

Note that in the process of writing this PR I discovered that the method
for determining whether to increment the signErrorCount metric is
broken. Rather than diverge the crlImpl's version of that code from the
identical code in the other two files, I have duplicated the broken code
and will fix it in all three places in a follow-up.

CA: Remove deprecated crldpBase config (#7461)
Remove the CA's global "crldpBase" config item, and the code which used
it to produce a IDP URI in our CRLs if it was configured.

This config item has been replaced by per-issuer crlURLBase configs
instead, because we have switched our CRL URL format from
"commonURL/issuerID/shard.crl" to "issuerURL/shard.crl" in anticipation
of including these URLs directly in our end-entity certs.

Add pkilint to CI via custom zlint (#7441)
Add a new "LintConfig" item to the CA's config, which can point to a
zlint configuration toml file. This allows lints to be configured, e.g.
to control the number of rounds of factorization performed by the Fermat
factorization lint.

Leverage this new config to create a new custom zlint which calls out to
a configured pkilint API endpoint. In config-next integration tests,
configure the lint to point at a new pkilint docker container.

This approach has three nice forward-looking features: we now have the
ability to configure any of our lints; it's easy to expand this
mechanism to lint CRLs when the pkilint API has support for that; and
it's easy to enable this new lint if we decide to stand up a pkilint
container in our production environment.

No production configuration changes are necessary at this time.

CA: Load multiple certificate profiles (#7325)
This change introduces a new config key `certProfiles` which contains a
map of `profiles`. Only one of `profile` or `certProfiles` should be
used, because configuring both will result in the CA erroring and
shutting down. Further, the singular `profile` is now
[deprecated](https://github.com/letsencrypt/boulder/issues/7414).

The CA pre-computes several maps at startup; 
* A human-readable name to a `*issuance.Profile` which is referred to as
"name".
* A SHA-256 sum over the entire contents of the given profile to the
`*issuance.Profile`. We'll refer to this as "hash".

Internally, CA methods no longer pass an `*issuance.Profile`, instead
they pass a structure containing maps of certificate profile
identifiers. To determine the default profile used by the CA, a new
config field `defaultCertificateProfileName` has been added to the
Issuance struct. Absence of `defaultCertificateProfileName` will cause
the CA to use the default value of `defaultBoulderCertificateProfile`
such as for the the deprecated `profile`. The key for each given
certificate profile will be used as the "name". Duplicate names or
hashes will cause the CA to error during initialization and shutdown.

When the RA calls `ra.CA.IssuePrecertificate`, it will pass an arbitrary
certificate profile name to the CA triggering the CA to lookup if the
name exists in its internal mapping. The RA maintains no state or
knowledge of configured certificate profiles and relies on the CA to
provide this information. If the name exists in the CA's map, it will
return the hash along with the precertificate bytes in a
`capb.IssuePrecertificateResponse`. The RA will then call
`ra.CA.IssueCertificateForPrecertificate` with that same hash. The CA
will lookup the hash to determine if it exists in its map, and if so
will continue on with certificate issuance.

Precertificate and certificate issuance audit logs will now include the
certificate profile name and hex representation of the hash that they
were issued with.

Add CRL capabilities to issuance package (#7300)
Move the CRL issuance logic -- building an x509.RevocationList template,
populating it with correctly-built extensions, linting it, and actually
signing it -- out of the //ca package and into the //issuance package.
This means that the CA's CRL code no longer needs to be able to reach
inside the issuance package to access its issuers and certificates (and
those fields will be able to be made private after the same is done for
OCSP issuance).

Additionally, improve the configuration of CRL issuance, create
additional checks on CRL's ThisUpdate and NextUpdate fields, and make it
possible for a CRL to contain two IssuingDistributionPoint URIs so that
we can migrate to shorter addresses.

IN-10045 tracks the corresponding production changes.

Separate issuance.Profile out from issuance.Issuer (#7285)
Remove the Profile field from issuance.Issuer, to reflect the fact that
profiles are in fact independent pieces of configuration which can be
shared across (and are configured independently of) multiple issuers.

Move the IssuerURL, OCSPUrl, and CRLURL fields from issuance.Profile to
issuance.Issuer, since they reflect fundamental attributes of the
issuer, rather than attributes of a particular profile. This also
reflects the location at which those values are configured, in
issuance.IssuerConfig.

All other changes are fallout from the above: adding a Profile argument
to various methods in the issuance and linting packages, adding a
profile field to the caImpl struct, etc. This change paves the way for
two future changes: moving OCSP and CRL creation into the issuance
package, and supporting multiple simultaneous profiles that the CA can
select between.

Prevent serial prefixes from beginning with a 1 (#7214)
Change the max value of the CA's `SerialPrefix` config value from 255 (a
byte of all 1s) to 127 (a byte of one 0 followed by seven 1s). This
prevents the serial prefix from ever beginning with a 1.

This is important because serials are interpreted as signed
(twos-complement) integers, and are required to be positive -- a serial
whose first bit is 1 is considered to be negative and therefore in
violation of RFC 5280. The go stdlib fixes this for us by prepending a
zero byte to any serial that begins with a 1 bit, but we'd prefer all
our serials to be the same length.

Corresponding config change was completed in IN-9880.

Simplify the features package (#7204)
Replace the current three-piece setup (enum of feature variables, map of
feature vars to default values, and autogenerated bidirectional maps of
feature variables to and from strings) with a much simpler one-piece
setup: a single struct with one boolean-typed field per feature. This
preserves the overall structure of the package -- a single global
feature set protected by a mutex, and Set, Reset, and Enabled methods --
although the exact function signatures have all changed somewhat.

The executable config format remains the same, so no deployment changes
are necessary. This change does deprecate the AllowUnrecognizedFeatures
feature, as we cannot tell the json config parser to ignore unknown
field names, but that flag is set to False in all of our deployment
environments already.

be able to live reload specific config files. Each location is now
responsible for loading the config file.

cmd: fail without panic (#6935)
For "ordinary" errors like "file not found" for some part of the config,
we would prefer to log an error and exit without logging about a panic
and printing a stack trace.

To achieve that, we want to call `defer AuditPanic()` once, at the top
of `cmd/boulder`'s main. That's so early that we haven't yet parsed the
config, which means we haven't yet initialized a logger. We compromise:
`AuditPanic` now calls `log.Get()`, which will retrieve the configured
logger if one has been set up, or will create a default one (which logs
to stderr/stdout).

AuditPanic and Fail/FailOnError now cooperate: Fail/FailOnError panic
with a special type, and AuditPanic checks for that type and prints a
simple message before exiting when it's present.

This PR also coincidentally fixes a bug: panicking didn't previously
cause the program to exit with nonzero status, because it recovered the
panic but then did not explicitly exit nonzero.

grpc/sa: Implement deep health checks (#6928)
Add the necessary scaffolding for deep health checking of our various
gRPC components. Each component implementation that also implements the
grpc.checker interface will be checked periodically, and the health
status of the component will be updated accordingly.

Add the necessary methods to SA to implement the grpc.checker interface
and register these new health checks with Consul.

Additionally:
- Update entry point script to check for ProxySQL readiness.
- Increase the poll rate for gRPC Consul checks from 5s to 2s to help
with DNS failures, due to check failures, on startup.
- Change log level for Consul from INFO to ERROR to deal with noisy logs
full of transport failures due to Consul gRPC checks firing before the
SAs are up.

Rename CA's "--ca-addr" flag to "--addr" (#6889)
Most boulder components have a command line flag to override what gRPC
and debug port they listen on, which is used in tests to run multiple
instances with the same configuration.

However, CA's flag is named "--ca-addr", and not "--addr". This is
inconsistent with SA, RA, VA, nonce, publisher, and purger.

This flag isn't used in production, where we set it in the config file,
so it shouldn't be a breaking change to rename it.

Introduce OpenTelemetry Tracing (#6750)
Add a new shared config stanza which all boulder components can use to
configure their Open Telemetry tracing. This allows components to
specify where their traces should be sent, what their sampling ratio
should be, and whether or not they should respect their parent's
sampling decisions (so that web front-ends can ignore sampling info
coming from outside our infrastructure). It's likely we'll need to
evolve this configuration over time, but this is a good starting point.

Add basic Open Telemetry setup to our existing cmd.StatsAndLogging
helper, so that it gets initialized at the same time as our other
observability helpers. This sets certain default fields on all
traces/spans generated by the service. Currently these include the
service name, the service version, and information about the telemetry
SDK itself. In the future we'll likely augment this with information
about the host and process.

Finally, add instrumentation for the HTTP servers and grpc
clients/servers. This gives us a starting point of being able to monitor
Boulder, but is fairly minimal as this PR is already somewhat unwieldy:
It's really only enough to understand that everything is wired up
properly in the configuration. In subsequent work we'll enhance those
spans with more data, and add more spans for things not automatically
traced here.

Deprecate ROCSPStage7 flag (#6804)
Deprecate the ROCSPStage7 feature flag, which caused the RA and CA to
stop generating OCSP responses when issuing new certs and when revoking
certs. (That functionality is now handled just-in-time by the
ocsp-responder.) Delete the old OCSP-generating codepaths from the RA
and CA. Remove the CA's internal reference to an OCSP implementation,
because it no longer needs it.

Additionally, remove the SA's "Issuers" config field, which was never
used.

Simplify how gRPC services start, stop, and clean up (#6771)
The CA, RA, and VA have multiple goroutines running alongside primary
gRPC handling goroutine. These ancillary goroutines should be gracefully
shut down when the process is about to exit. Historically, we have
handled this by putting a call to each of these goroutine's shutdown
function inside cmd.CatchSignals, so that when a SIGINT is received, all
of the various cleanup routines happen in sequence.

But there's a cleaner way to do it: just use defer! All of these
cleanups need to happen after the primary gRPC server has fully shut
down, so that we know they stick around at least as long as the service
is handling gRPC requests. And when the service receives a SIGINT,
cmd.CatchSignals will call the gRPC server's GracefulStop, which will
cause the server's .Serve() to finally exit, which will cause start() to
exit, which will cause main() to exit, which will cause all deferred
functions to be run.

In addition, remove filterShutdownErrors as the bug which made it
necessary (.Serve() returning an error even when GracefulShutdown() is
called) was fixed back in 2017. This allows us to call the start()
function in a much more natural way, simply logging any error it returns
instead of calling os.Exit(1) if it returns an error.

This allows us to simplify the exit-handling code in these three
services' main() functions, and lets us be a bit more idiomatic with our
deferred cleanup functions.

Delete the ocsp-updater service, and the //ocsp/updater library that
supports it. Remove test configs for the service, and remove references
to the service from other test files.

This service has been fully shut down for an extended period now, and is
safe to remove.

- Require `letsencrypt/validator` package.
- Add a framework for registering configuration structs and any custom
validators for each Boulder component at `init()` time.
- Add a `validate` subcommand which allows you to pass a `-component`
name and `-config` file path.
- Expose validation via exported utility functions
`cmd.LookupConfigValidator()`, `cmd.ValidateJSONConfig()` and
`cmd.ValidateYAMLConfig()`.
- Add unit test which validates all registered component configuration
structs against test configuration files.

CA: Stop gRPC service before helper goroutines (#6761)
When shutting down the CA, we should stop the primary gRPC service
(which waits for any outstanding requests to complete) before shutting
down the other helper goroutines. This prevents panics when an ongoing
gRPC request attempts to write to the OCSPLogQueue, but the queue's
channel has already been closed by the call to ocspi.Stop().

Remove beeline tracing (#6733)
Remove tracing using Beeline from Boulder. The only remnant left behind
is the deprecated configuration, to ensure deployability.

We had previously planned to swap in OpenTelemetry in a single PR, but
that adds significant churn in a single change, so we're doing this as
multiple steps that will each be significantly easier to reason about
and review.

CA: Remove secondary gRPC servers (#6496)
Remove the OCSPGenerator and CRLGenerator gRPC servers that run on
separate ports from the CA's main gRPC server, which exposes both those

Move cmd.ConfigDuration to config.Duration (#6705)
We rely on the ratelimit/ package in CI to validate our ratelimit
configurations. However, because that package relies on cmd/ just for
cmd.ConfigDuration, many additional dependencies get pulled in.

This refactors just that struct to a separate config package. This was
done using Goland's automatic refactoring tooling, which also organized
a few imports while it was touching them, keeping standard library,
internal and external dependencies grouped.

Remove OCSP and CRL methods from CA gRPC service (#6474)
Remove the GenerateOCSP and GenerateCRL methods from the
CertificateAuthority gRPC service. These methods are no longer called by
any clients; all clients use their respective OCSPGenerator and
CRLGenerator gRPC services instead.

In addition, remove the CRLGeneratorServer field from the caImpl, as it
no longer needs it to serve as a backing implementation for the
GenerateCRL pass-through method. Unfortunately, we can't remove the
OCSPGeneratorServer field until after ROCSPStage7 is complete, and the
CA is no longer generating an OCSP response during initial certificate
issuance.

CA: Expose all gRPC services on single address (#6495)
Now that we have the ability to easily add multiple gRPC services to the
same server, and control access to each service individually, use that
capability to expose the CA's CertificateAuthority, OCSPGenerator, and
CRLGenerator services all on the same address/port. This will make
establishing connections to the CA easier, but no less secure.

bgrpc.NewServer: support multiple services (#6487)
Turn bgrpc.NewServer into a builder-pattern, with a config-based
initialization, multiple calls to Add to add new gRPC services, and a
final call to Build to produce the start() and stop() functions which
control server behavior. All calls are chainable to produce compact code
in each component's main() function.

This improves the process of creating a new gRPC server in three ways:
1) It avoids the need for generics/templating, which was slightly
verbose.
2) It allows the set of services to be registered on this server to be
known ahead of time.
3) It greatly streamlines adding multiple services to the same server,
which we use today in the VA and will be using soon in the SA and CA.

While we're here, add a new per-service config stanza to the
GRPCServerConfig, so that individual services on the same server can
have their own configuration. For now, only provide a "ClientNames" key,
which will be used in a follow-up PR.

CA: Add configs to disable each gRPC service (#6473)
Add three new boolean config keys to the CA's config json:
- DisableCertService
- DisableOCSPService
- DisableCRLService

Setting any one of these flags to True and restarting the CA causes the
corresponding gRPC service (CertificateAuthority, OCSPGenerator, or
CRLGenerator, respectively) to not be started. The implementation is not
instantiated, the port is not opened, and no requests will be listened
for. All clients will receive failures to connect.

Also, create two temporary "dummy" implementations of the CRL and OCSP
services (and an interface for the fake OCSP service to meet), These are
necessary because the CertificateAuthority gRPC service currently
contains wrapper methods which replicate the methods of the OCSP and CRL
services, and which pass through calls to underlying implementation
objects. These objects must be replaced with fake objects which always
return an error when the service is disabled.

Streamline gRPC client creation (#6472)
Remove the need for clients to explicitly call bgrpc.NewClientMetrics,
by moving that call inside bgrpc.ClientSetup. In case ClientSetup is
called multiple times, use the recommended method to gracefully recover
from registering duplicate metrics. This makes gRPC client setup much
more similar to gRPC server setup after the previous server refactoring
change landed.

Streamline gRPC server creation (#6457)
Collapse most of our boilerplate gRPC creation steps (in particular,
creating default metrics, making the server and listener, registering
the server, creating and registering the health service, filtering
shutdown errors from the output, and gracefully stopping) into a single
function in the existing bgrpc package. This allows all but one of our
server main functions to drop their calls to NewServer and
NewServerMetrics.

To enable this, create a new helper type and method in the bgrpc
package. Conceptually, this could be just a new function, but it must be
attached to a new type so that it can be generic over the type of gRPC
server being created. (Unfortunately, the grpc.RegisterFooServer methods
do not accept an interface type for their second argument).

The only main function which is not updated is the boulder-va, which is
a special case because it creates multiple gRPC servers but (unlike the
CA) serves them all on the same port with the same server and listener.

CRLs: include IssuingDistributionPoint extension (#6412)
Add the Issuing Distribution Point extension to all of our end-entity
CRLs. The extension contains the Distribution Point, the URL from
which this CRL is meant to be downloaded. Because our CRLs are
sharded, this URL prevents an on-path attacker from substituting a
different shard than the client expected in order to hide a revocation.
The extension also contains the OnlyContainsUserCerts boolean,
because our CRLs only contain end-entity certificates.

The Distribution Point url is constructed from a configurable base URI,
the issuer's NameID, the shard index, and the suffix ".crl". The base
URI must use the "http://" scheme and must not end with a slash.

openssl displays the IDP extension as:

Add CRLDPBase config key to boulder-ca (#6442)
Add a new configuration key to the CA which allows us to
specify the "base URL" for our CRLs. This will be necessary
before including an Issuing Distribution Point extension in our
CRLs, or a CRL Distribution Point in our certificates.
