This is work towards resolving the response.finished confusion and
future deprecation.
Note that implementation-wise, streams have both an ending and ended
state. However, in this case (in order to avoid confusion in user space)
writableEnded is equal to writable.ending. The ending vs ended situation
is internal state required for internal stream logic.

This switches all `util.inherits()` calls to use

Make sure that it is safe to call the callback for `_write()`
even in the presence of `.destroy()` calls during that write.
In particular, letting the write queue continue processing would
previously have thrown an exception, because processing writes
after calling `.destroy()` is forbidden.
One test had to be modified to account for the fact that callbacks
for writes will now always be called, even when the stream
is destroyed during the process.

This improves error handling for streams in a few ways.
1. It ensures that no user defined methods (_read, _write, ...) are run
after .destroy has been called.
2. It introduces an explicit error to tell the user if they are write to
write, etc to the stream after it has been destroyed.
3. It makes streams always emit close as the last thing after they have
been destroyed
4. Changes the default _destroy to not gracefully end streams.
It also updates net, http2, zlib and fs to the new error handling.

This adds computed properties to readable and writable streams to
allow access to the readable buffer, the writable buffer, and flow
state without accessing the readable or writable state.
These are the only uses of readable and writable state in the docs
so adding these work arounds allows them to be removed from the docs.
This also updates net, http_client and http_server to use the new
methods instead of manipulating readable and writable state directly.

`validChunk` allowed `undefined` as a chunk in object mode; however,
this was redundant, since:
- `validChunk()` is only used by `.write()`
- If the `validChunk()` check passes for `undefined`, `.write()`
calls `writeOrBuffer()`
- If `writeOrBuffer()` does not receive a Buffer, it calls
`decodeChunk()`
- `decodeChunk()` ignores `undefined` because it checks
`typeof chunk === 'string'`
- After that call, `chunk.length` is accessed, which throws an
error if `chunk` is undefined`.
This “fixes” a bug in the sense that `state.pendingcb` is no longer
incremented for write attempts that fail like this.

When _write completes with an Error, 'finish' was emitted before
'error' if the callback was asynchronous. This commit restore the
previous behavior.
The logic is still less then ideal, because we call the write()
callback before emitting error if asynchronous, but after if
synchronous. This commit do not try to change the behavior.

Adds the ability to for write streams to have an _final method which acts
similarly to the _flush method that transform streams have but is called before
the finish event is emitted and if asynchronous delays the stream from
finishing. The `final` option may also be passed in order to set it.

In most cases, named functions match the variable or property to which
they are being assigned. That also seems to be the practice in a series
of PRs currently being evaluated that name currently-anonymous
functions.
This change applies that rule to instances in the code base that don't
comply with that practice.
This will be enforceable with a lint rule once we upgrade to ESLint
3.8.0.

2a4b068aca introduced a regression in where checking
`instanceof` would fail for `Writable` subclasses inside the
subclass constructor, i.e. before `Writable()` was called.
Also, calling `null instanceof Writable` or
`undefined instanceof Writable` would fail due to accessing the
`_writableState` property of the target object.
This fixes these problems.

These properties were initially used to determine stream status
back in node v0.8 and earlier. Since streams2 however, these
properties were *always* true, which can be misleading for
example if you are trying to immediately determine whether
a Writable stream is still writable or not (to avoid a "write after
end" exception).

1. Making the deprecation messages consistent. The messages will be in
the following format
x is deprecated. Use y instead.
If there is no alternative for `x`, then the ` Use y instead.` part
will not be there in the message.
2. All the internal deprecation messages are printed with the prefix
`(node) `, except when the `--trace-deprecation` flag is set.

Many of the util.is*() methods used to check data types
simply compare against a single value or the result of
typeof. This commit replaces calls to these methods with
equivalent checks. This commit does not touch calls to the
more complex methods (isRegExp(), isDate(), etc.).

The copyright and license notice is already in the LICENSE file. There
is no justifiable reason to also require that it be included in every
file, since the individual files are not individually distributed except
as part of the entire package.
I was originally going to do this after the v0.11.15 release, but as
that release is three weeks overdue now, I decided not to wait any
longer; we don't want the delta to get too big.

In cases where many small writes are made to a stream
lacking _writev, the array data structure backing the
WriteReq buffer would greatly increase GC pressure.
Specifically, in the fs.WriteStream case, the
clearBuffer routine would only clear a single WriteReq
from the buffer before exiting, but would cause the
entire backing array to be GC'd. Switching to [].shift
lessened pressure, but still the bulk of the time was
spent in memcpy.
This replaces that structure with a linked list-backed
queue so that adding and removing from the queue is O(1).
In the _writev case, collecting the buffer requires an
O(N) loop over the buffer, but that was already being
performed to collect callbacks, so slowdown should be
neglible.
