This implements a flavor of vm.createContext() and friends
that creates a context without contextifying its global object.
This is suitable when users want to freeze the context (impossible
when the global is contextified i.e. has interceptors installed)
or speed up the global access if they don't need the interceptor
behavior.

const context = vm.createContext(vm.constants.DONT_CONTEXTIFY);

This patch adds support for using
`vm.constants.USE_MAIN_CONTEXT_DEFAULT_LOADER` as
`importModuleDynamically` in all APIs that take the option
except `vm.SourceTextModule`. This allows users to have a shortcut
to support dynamic import() in the compiled code without missing
the compilation cache if they don't need customization of the
loading process. We emit an experimental warning when the
`import()` is actually handled by the default loader through
this option instead of requiring `--experimental-vm-modules`.
In addition this refactors the documentation for
`importModuleDynamically` and adds a dedicated section for it
with examples.
`vm.SourceTextModule` is not supported in this patch because
it needs additional refactoring to handle `initializeImportMeta`,
which can be done in a follow-up.

1. Enforce receiver checks on IDL interfaces.
2. Avoid prototype manipulation on constructing IDL interfaces with
`ReflectConstruct`.
3. `defineReplaceableAttribute` should create IDL getter/setter.
4. Corrected `PerformanceResourceTiming` to inherit the public interface
`PerformanceEntry` instead of the internal interface
`InternalPerformanceResourceTiming`.
5. `detail` is not a specified attribute on `PerfomanceEntry`. Node.js
specific extensions are moved to a subclass of `PerformanceEntry` as

This commit documents the event parameters and `http2stream.respond`,
and adds some tests to ensure the actual behaviors are aligned with
the docs.
Testing the 'Http2Server.sessionError' event is added by updating
`test/parallel/test-http2-options-max-headers-exceeds-nghttp2.js`.
The event seemingly has not been tested so far.
`ServerHttp2Session` is exported to validate the `session` event
and the `sessionError` event.

Main changes:
- Replace current HTML anchor generation to match
header anchor generation in Github markdown.
- Remove unnecessary double namespacing on generated anchors/links (E.G.
`esm.md#loaders` instead of `esm.md#esm_loaders`).
- Anchors/links are automatically prefixed with their respective modules
when concatenated for usage in `all.html`.
Benefits:
- All anchor links within and between markdown API docs actually work.
- Adding new anchor links no longer requires contributors to generate
the HTML docs first to look up the correct anchors.
- Anchors are much shorter.
- All previous anchor links are preserved by generating hidden legacy
anchors.
