Generate instrumentation packages setup.py files (#474) 
All instrumentations packages have almost exactly same setup.py files.
This commit adds a python script that generates it from a source
template. This dramatically reduces the time and effort required to
update all instrumentation setup.py files, and also reduces chances of
making manual mistakes.

OpenTelemetry Pyramid Instrumentation

Installation
------------

::

    pip install opentelemetry-instrumentation-pyramid

Exclude lists
*************
To exclude certain URLs from being tracked, set the environment variable ``OTEL_PYTHON_PYRAMID_EXCLUDED_URLS`` with comma delimited regexes representing which URLs to exclude.

For example, 

::

    export OTEL_PYTHON_PYRAMID_EXCLUDED_URLS="client/.*/info,healthcheck"

will exclude requests such as ``https://site/client/123/info`` and ``https://site/xyz/healthcheck``.
