Contrib guide: more PR and review info and some cleanups
This PR has a series changes to the new contributing guide spanning:
* add a more cohesive PR section
* add a more cohesive code review section
* misc cleanups
I'm not sure that some of these are correct, so I split them into separate patches to allow more specific review and ack/nack.
This patch hits a couple of improvement needed areas:
* add a pull requests section
* add a code review section
* misc smaller cleanups noticed while doing the prior two

contributing guide: add initial section on testing
This commit replaces the "Improvements needed" marker for the testing
topic with an initial section of overview information and jumping off links
to sig-testing, test documentation, and test code.
